Mediterranean Diet
Vegetables, olive oil, and fish — one of the most rigorously studied eating patterns for heart health.
How It Works
The Mediterranean diet isn't a rulebook with hard limits — it's a pattern modeled on how people
historically ate around the Mediterranean basin, formalized in the Oldways/Harvard/WHO
Mediterranean Diet Pyramid (1993). At its base: vegetables, fruit, whole grains,
legumes, nuts, seeds, herbs, and extra-virgin olive oil as the primary fat, alongside
daily physical activity and water. One step up: fish and seafood at least twice a week. Above that:
eggs, poultry, and dairy (yogurt, cheese) in moderate daily-to-weekly amounts. At the top, eaten only a
few times a month: red meat, processed meat, and sweets.
There's no calorie counting or macro ratio built into the pattern — it's a food-quality and food-choice
framework, not a weight-loss protocol. Total calories still determine whether you lose, maintain, or
gain weight on it, same as any other way of eating.
The Research
This is one of the most rigorously studied diets in nutrition science, with decades of both randomized-trial and large cohort evidence linking it to reduced cardiovascular disease, stroke, type 2 diabetes risk, and all-cause mortality.

"Primary Prevention of Cardiovascular Disease with a Mediterranean Diet Supplemented with Extra-Virgin Olive Oil or Nuts" (PREDIMED Trial), New England Journal of Medicine, 2018. A 7,447-person Spanish trial finding roughly 30% fewer major cardiovascular events in the Mediterranean-diet groups versus a low-fat control. Pooling 12 cohort studies (~1.57 million people), each 2-point increase in diet adherence was linked to 9% lower all-cause mortality, 9% lower cardiovascular mortality, and 6% lower cancer incidence and mortality.

Salas-Salvadó J, Bulló M, Estruch R, et al. "Prevention of Diabetes With Mediterranean Diets: A Subgroup Analysis of a Randomized Trial," Annals of Internal Medicine, 2014. In a PREDIMED subgroup of 3,541 nondiabetic adults, the Mediterranean diet supplemented with olive oil (with no calorie restriction) cut new-onset type 2 diabetes versus a control diet. Worth knowing: the parallel nuts-arm reduction in this same study did not reach statistical significance — the benefit wasn't uniform across both versions of the diet.

Food Examples
Emphasized: vegetables (tomatoes, leafy greens, eggplant, peppers), fruit (figs,
citrus, grapes), legumes (chickpeas, lentils), whole grains (bulgur, farro, whole-wheat pasta), nuts
and seeds (almonds, walnuts), extra-virgin olive oil, fatty fish (sardines, salmon, mackerel), and
herbs (oregano, garlic).
Limited: red and processed meat (sausage, bacon), butter and margarine, refined
grains, sugary drinks, packaged sweets, and highly processed snacks.
A Sample Day
Breakfast: plain yogurt with walnuts and berries.
Lunch: chickpea-and-vegetable salad with olive oil and feta, whole-grain bread.
Snack: an apple with almonds.
Dinner: grilled salmon, sautéed greens, farro, and a side salad.
Who It Might Suit
Tends to suit people focused on long-term cardiovascular and metabolic health who prefer a flexible, food-based pattern over strict rules or tracking. It may be less practical for tight food budgets or limited access to fresh produce and fish, strong aversions to fish or olive oil, or cultural food contexts where the reference foods aren't the norm. It also has no built-in weight-loss mechanism — total calories still determine that outcome — so anyone with a specific weight target will want to pair it with a calorie or macro target. Fish, shellfish, or tree-nut allergies require substitution.
