Carnivore Diet
An all-animal-food elimination diet with strong short-term anecdotes but essentially no long-term clinical evidence.
What Is the Carnivore Diet?
The carnivore diet eliminates every plant food and eats only animal products — meat, fish, eggs, and for some followers a limited amount of dairy. Some stricter versions (sometimes called the "lion diet") narrow further to just meat, salt, and water. There's no single official rulebook or governing organization behind it, so exact versions vary from one follower to the next.
Most people who try it describe a health motivation rather than a performance or environmental one: in the largest self-reported survey of carnivore dieters to date, 93% cited health reasons for starting. The appeal is largely framed as an elimination diet taken to its logical extreme — remove essentially everything except meat, and any symptom that improves must have been caused by something you cut out. It's also credited anecdotally with relief from autoimmune and IBS-type digestive symptoms, though that's self-reported and anecdotal rather than something clinical trials have confirmed.
How It Works
There's no macro target or calorie structure the way there is with keto or flexible dieting — the entire "rule" is the exclusion list. Fruits, vegetables, grains, legumes, nuts, seeds, and added sugar are all removed, leaving meat (often including organ meats like liver), fish, eggs, and sometimes butter, cheese, or other dairy. Because virtually all carbohydrate is removed along with the produce, most followers end up in ketosis by default, similar to keto, but without keto's more moderate protein target or any plant-food allowance at all.
Because the diet removes entire food groups rather than following a specific macro or calorie formula, there's also no standard "compliant vs. non-compliant" list the way Whole30 has — individual followers decide for themselves whether dairy, honey, or certain spices count as acceptable exceptions.
The Evidence
There are no randomized controlled trials of the carnivore diet. A 2026 scoping review in Nutrients — the most comprehensive evidence assessment done on the diet so far — identified only nine human studies published between 2021 and 2025, and every one of them was observational: five case studies, two social-media surveys, one modeling study, and one exploratory study, with no random assignment and no control group in any of them. The review rated the overall evidence quality as NHMRC Levels III–IV — the lowest tiers on that evidence hierarchy — and concluded that "it is not currently possible to reliably assess the long-term safety" of the diet, adding that long-term adherence "cannot be recommended" given how little is actually known.

The largest single study is a self-reported social-media survey of 2,029 adults who said they'd followed a carnivore diet for at least six months (median 14 months). Respondents reported high satisfaction and improvement across a range of self-assessed conditions, and among the subset who reported lab values, average LDL cholesterol was markedly elevated (172 mg/dL) while HDL (68 mg/dL) and triglycerides (68 mg/dL) looked favorable — a mixed lipid pattern sometimes seen on very-low-carb, high-fat diets more broadly. But this is self-reported data from a self-selected group of committed carnivore dieters recruited through social media, with no independent verification of the lab values — a limitation other researchers have specifically challenged in a published response, and one the original authors partially conceded while defending the plausibility of the numbers.

Independent assessment: Harvard T.H. Chan's Walter Willett has called the diet "basically a terrible idea," and Cleveland Clinic's dietitian-reviewed overview lists heart disease, colorectal cancer risk from high red-meat intake, and nutrient deficiencies among its concerns. Neither source found evidence supporting the diet's health claims beyond short-term weight loss from cutting sugar and refined carbs.

Nutritional Concerns / Who Should Be Cautious
Zero dietary fiber. Removing every plant food removes 100% of dietary fiber, which has well-established evidence behind it for cardiovascular and metabolic health — see our fiber page for the research on soluble/insoluble fiber's role in cholesterol, blood sugar, and gut health. The 2026 scoping review found carnivore dieters' fiber intake typically fell below 1g per day against a 25–30g recommended adequate intake — essentially a complete elimination, not just a reduction.
Micronutrient gaps. The same review flagged deficiency risk across thiamine, vitamin C, vitamin D, magnesium, calcium, iron, iodine, and folate — nutrients concentrated in the fruits, vegetables, and grains the diet removes entirely. Vitamin C and folate are worth flagging specifically since they have essentially no significant carnivore-compatible sources at all beyond small amounts in organ meats.
Documented scurvy risk. This isn't hypothetical: a published case report describes a man who developed clinically confirmed scurvy on two separate occasions roughly two years apart after eating an almost entirely meat-based diet, going through extensive misdiagnosis both times before vitamin C deficiency was recognized as the cause.

Cardiovascular risk from saturated fat. The diet's near-total reliance on red meat and animal fat pushes saturated fat intake well past the WHO's recommended upper limit, and the scoping review reported elevated total and LDL cholesterol across the available studies — a real, mechanistically-understood cardiovascular risk factor. The picture isn't uniformly bad: some self-reported carnivore dieters show a "mixed" lipid pattern with high LDL alongside favorable HDL and triglycerides, and proponents point to that pattern as reassuring. But that data comes from unverified self-report in a self-selected group, not from trials measuring actual cardiovascular outcomes, so the long-term risk from sustained high LDL on this diet remains genuinely unresolved either way.
Other flagged risks. The scoping review and independent reviews also note increased colorectal cancer risk associated with high red-meat intake generally, kidney stress and kidney stone risk from very high animal protein intake (documented in at least one case in the review), and digestive issues like constipation from the complete absence of fiber.
People with kidney disease, a history of kidney stones, or existing high LDL cholesterol should be especially cautious, as should vegetarians and vegans for obvious reasons. Anyone considering it long rather than as a short, medically-supervised elimination experiment should know that essentially no research exists on what happens beyond a year or two of adherence.
